UltiWellness Solutions ("we", "us") provides nutrition counseling in Atlanta, Georgia and virtually. This policy explains what information this website collects and how we use it. It covers this website only. It is not a Notice of Privacy Practices for clinical care, and it does not describe how we handle health records once you become a client.
Information we collect
If you use our contact form, we receive the details you enter: your name, email address, optional phone number, the service you are interested in, whether you expect to pay privately or use insurance, how you prefer to be contacted, and any message you choose to write. We also record that you agreed to be contacted.
If you email or call us, we receive whatever you share in that email or call.
Please do not send private medical information through this website
The contact form and ordinary email are not designed for medical records. Please do not include diagnoses, lab results, medications or other private health details in the form or in a regular email. Angela will discuss those with you directly after you reach out.
How we use it
We use your information only to respond to your request, to understand your goals, to discuss cost and coverage, and to schedule care if you decide to proceed. We do not sell your information, and we do not use it for advertising.
Who handles it for us
- Netlify hosts this website and receives contact form submissions on our behalf. Submissions are forwarded to our practice email, which is hosted by Google Workspace.
- Stripe processes invoice payments on its own secure site. When you choose "Pay an invoice", you leave this website and Stripe's terms and privacy policy apply. We do not collect or store your card details.
- Social links (such as Instagram, TikTok and Facebook) take you to those services, which have their own policies.
Cookies and tracking
This website does not use advertising or analytics cookies. Our hosting provider may keep standard server logs, such as the pages requested and technical details of the visit, to keep the site secure and working.
How long we keep it
We keep website inquiries only as long as needed to follow up and to keep a reasonable business record. You can ask us to delete your inquiry at any time.
Your choices
You can ask us what we received from you, ask us to correct it, or ask us to delete it by emailing info@myultiwellness.com or calling (404) 689-0015 ext. 800.
Children
This website is intended for adults. If you are contacting us about a child, a parent or guardian should submit the request.
Changes
If we change this policy, we will post the new version here and update the date above.
